Mildred Ratched was a nurse employee who ran a mental institution at Salem, Oregon. Inside there, any patient brought to her are to be treated by her own methods of treatment, the latter results turning out to be horrific for the mentally challenged and ill as she tortures them in punishment, whenever they dislike her methods and get far worse ones if they continue to go against her. This technique makes her patients obedient into doing what she says. She is considered a successful villain since she enraged the main character Randle McMurphy into violently attacking her into a choke hold by indirectly having his closest friend Billy Bibbit commit suicide by blackmailing him into telling his mom after he finally experienced the joys of being with a girl for the first time that he is always nervous to ask out on dates. Nurse Mildred Ratched was saved thanks to her attendant Washington who overpowered Randle and brought him to be strapped to a table where his brain would be lobotomized by her medical tools. Did she kill him directly? No, in fact Randle McMurphy lived as a patient in a emotionless and silent state, because of the dehumanization from the lobotomy surgery the nurse caretaker brought to him. The only time McMurphy died was when his Indian friend Chief Bromden visited him and saw him in this way and decided to make the escape plan with him, be on his own as Bromden puts Randle McMurphy out of his misery by suffocating him inside his pillow, killing him and allowing sweet release of his soul into heaven. It was not until the next night, Nurse Mildred Ratched was visited by Johan Liebert, who had Kakuzu to accompany him as he had read the newspaper headline of her losing her ability to speak after surviving the killing attempt made on her by her patient. Johan Liebert removes the neck brace covering her and orders Kakuzu to use his grey sewing threads to fix her vocal cords together. With the injury repaired, Nurse Mildred Ratched graciously thanks Johan Liebert, by telling him how she can repay him?
