Mad Rat is a lab rat whom died while he was vivisected by a doctor, but was given the ability to turn back time from the Rat God, so he can relive his final day and fulfill any final wishes he had. Mad Rat's wish though was to get revenge on the doctor who kept him in a cage for all of his life, by killing him.

Mad Rat is accompanied by Heart, which is Mad Rat's actual heart who somehow gained the ability to talk to Mad Rat, and after a while Mad Rat did eventually warm up to Heart. Heart also has the ability to generate music which gives Mad Rat his power, though it is shown that sometimes music appears without Heart which still gives Mad Rat power.

Mad Rat's abilities are quite limited, though he is quite powerful due to the fact that when he dies he can always time travel to any point before he died, though if Mad Rat faces off against a more powerful time manipulator, they can render his attempts at time travel useless.
